1.  Depending on the care regimen determined by the medical practitioner, “face to face” examinations will cost from $300 to $340. The charges for any telephone “follow up” consultations will be determined by the timing and frequency of those consultations as determined by the practitioner, but they are significantly cheaper than the “face to face” exams.
   
   DBVIP discounts will apply.
   
   The type, number and timing of consultations is entirely up to the professional decision of the practitioner.
   
   
2. From time to time, we may have to change prices or add items.  The new info will be shown on this page. In particular the special rules applying with respect to some states may require a different consultation fee for those states.

Note: In all COD cases, a $10 C.O.D. fee will be added.
